Bingo Approval Applications
Bingo licenses/permits are issued by the State with city reviewing the proposal and providing a "local governing body endorsement".
Step One
- Complete the state's Bingo Application
(pdf/481kb/5pp) - Call the Arizona Department of Revenue at 602-716-7801
Step Two
- Submit COS Bingo Application (pdf/68kb/1p) to Teri Gleason, in Development Services.
Step Three
- Bingo application will be reviewed by the City Council and a recommendation of approval or denial will be forwarded to Arizona Department of Revenue, Bingo Section.
