The results of this study will be used in reports, presentations, and publications, but your name and email will not be used.
Your participation in this inventory program is voluntary. If you choose not to participate or to withdraw from participating, you can do so at any time without any penalty. You must be 18 years or older to complete the feedback form or have the feedback form completed by an adult (parent or legal guardian) on your behalf.
Completing and submitting the feedback questions will serve as your consent to participate.
